Advance Care Planning Interoperability

Our secure, interoperable, cloud-based platform employs the most current industry standards when responding to outside requests from Health Information Exchanges (HIEs), Electronic Medical Record (EMR) systems, and data exchange partners.

HIEs Make the Connection

Although healthcare data transport standards are similar throughout the world, not every health information system can “talk” directly to each other. Health Information Exchanges (HIEs) are hubs that connect those systems that can’t talk directly.

ADVault is a member of or has a commercial agreement in place with these HIEs that allow us to share advance care planning documents anytime, anywhere, with nearly everyone in the country.

Kno2 Partnership

In November 2021 ADVault and Kno2 announced formation of an innovative partnership. Renowned for its ability to join participants simply and securely across the care continuum, the Kno2 Connected™ product provides the critical link to patient ACPs in the ADVault Exchange™. This fully integrated approach bridges potentially harmful communication gaps between hospital systems, emergency medical services, and post-acute care providers and the patients they serve in urgent care situations.

By virtue of our partnership with Kno2, ADVault became a part of the Carequality interoperability framework which benefits health data exchange networks from accelerated, less costly health data sharing agreements. Carequality enables nationwide care coordination amongst:

  • 600,000 providers
  • 50,000 clinics
  • 4,200+ hospitals
